Feedback does not apply to provider (Dr. Perez). Have never seen Dr. Perez- come to practice for shots only. Feedback applies only to non-provider staff. They are rude and discourteous. They have refused to even say hello to me when I greet them, and have done so for the past 5 weeks. They ask patients to give PHI (date of birth) on the sign in sheet that other patients can see- this is a HIPAA violation. Staff take unduly long to find serum and prepare for injections (10+ minutes) when few or no other patients are present. Guest Wi-Fi in practice has been down for over a month and staff have taken no action in response to patient complaints.
